PC8 Application Form Instructions
Children under the age of 18 years will be issued with individual passports valid for 5 years. Inclusion in another person's passport (e.g. parent) is not possible.
A passport application for a child must be completed by a parent or other person with a parental responsibility for the child. The person lodging the application should provide proof of their identity.
A checklist for completing applications for children is available to assist you.
If you are aged 75 years or over, you have the choice to apply for either a senior's passport with 5 years validity which will cost a lower fee, or a 10 year passport with the normal fee.
Your previous passport will be cancelled and returned to you with your new passport. Please include a self-addressed, stamp registered envelope when submitting your application. You should check the status of any visas in your passport with the authority which issued them as we are unable to provide any information about the status of visas.
PHOTOGRAPHS / SIGNATURES
Photographs must meet the guidelines as set out on the application form. We encourage you to show the diagram to the photographer. Photos taken in a photo booth or on a home digital camera will not be accepted. Your signature must be in black ink and within the white box area - there should be nothing outside the box including a loop or tail. Unacceptable photographs and / or signatures will be rejected.

The Geneva office operates an appointment system for personal lodgement of applications by applicants who fall into one of the categories listed below. LODGING YOUR FORM
The following passport applicants must lodge their applications in person at the Geneva office:
- First passport (children aged under 18 do not have to accompany lodging parent)
- First adult passport
- Applicants replacing a lost, stolen or seriously damaged travel document
- Applicants replacing limited validity travel documents (this includes emergency passports) and passports which were issued before 20 August 1986 and with an original validity of less than 2 years
- Applicants replacing a passport issued prior to 20 August 1986
- Applicants applying for their first passports on resumption of Australian citizenship
ALL OTHER APPLICATIONS MAY BE SUBMITTED BY REGISTERED MAIL
Applications forms which are incomplete or incorrect (e.g. unacceptable photographs) and those containing white-out and significant alterations are not acceptable.
These forms and supporting documents will be returned to you by registered mail for which a charge of CHF6 applies.
Adult applicants who are providing a citizenship certificate as proof of citizenship which does not details their date and place of birth, must provide their original birth certificate, officially translated into English where necessary. We are happy to accept the international version of your birth certificate which can be obtained from the issuing authority.
